Become A New Patient

There are a few steps to complete before scheduling your first appointment. These steps help Dr Kilthau spend less time on administrative tasks and more time caring for his patients. An inital deposit of $100 will be charged prior to your appointment. This deposit will be applied to the first visit payment, and is fully refundable if the appointment is cancelled with 24 hour notice. Also please note that at this time Dr Kilthau typically only has openings on Monday afternoons.

1. Complete the Request a Consultation Form (click below)

2. Once approved, use your online Patient Portal to 1) sign the Initial Visit Forms and 2) enter your payment information that will be used for the initial deposit.

3. Next you will receive a message that you are ready to schedule your first visit with Dr Kilthau using the Patient Portal.  New patient appointments are usually available within 1-3 weeks.
